Attendance Form

To help us better organize and track attendance documentation, we are streamlining the way we collect student absence notes. Moving forward, all notes will be submitted through a Google Form so they are received directly by our data manager.

If your student is absent due to illness, injury, or other reasons, please complete the form to notify us. The form also includes an option to upload doctor’s notes or other supporting documentation for your convenience.
