Health Information for Kindergarten
Welcome to Turner Creek Year Round Elementary. The school nurse's name for our school is Barbara Cullen. Please be aware of the following policies related to your child’s health care at school. We appreciate your support with these responsibilities.
1.
Your
child must have a complete physical before entering kindergarten. This physical must have been done within one
year of the starting date of school for your child. If this is not in place within 30 days of school starting, the
principal will be required by law to suspend your child. This is not a 5-year-old physical. It is a pre kindergarten physical and can be
done any time after your child turns 4 years old. If you cannot get this done prior to the start of school, you
must submit to the school an official note from a health care provider that you
do have an appointment and that you are in the process of taking care of this
responsibility.

During
this physical your child should complete their immunizations. There are several boosters that they can
only receive on or after their 4th birthday in order to be in
compliance with NC law. Failure to have
these on record within 30 days of the start of school can result in suspension.
3.
If your child needs to be on any type of
medication at school, please stop by the school office and pick up a medication
form. You will need to fill this out,
take it to your child’s physician to complete, and bring it to the office with
the medication in an appropriately labeled bottle.
4.
If
your child has a medical problem that will need specific interventions at
school, please contact Barbara Cullen, and she will work on creating an
appropriate care plan for your child.
Medical problems would include such illnesses as diabetes, severe
allergies (EPIPEN), severe asthma, or any other chronic illnesses that have the
potential to impact your child at school.
